Each insurance plan is different. Provided below are the most
common plans and their requirements that we see. We have also provided
a link to each plan. Please contact your carrier for the details
about your specific plan.

Insurance Plans
Blue Cross/ Blue Shield
- BMI of 40+ or 35 with co-morbidities (Diabetes, Hypertension,
Osteoarthritis, etc.) You must bring supporting medical records
from your primary medical doctor
- 18+ years of age-
- Medical
and Diet history sheet that you fill out in office during
consultation or print and bring with you
- Documented weight for past 3 years
- 6 consecutive months of weight loss documentation. If months
are skipped, you have to start over
- Dietary Consultation
- For our use: Psychiatric Evaluation and *H. Pylori
Breath Test
Federal Blue Cross
- Only the first three requirements of BC/BS
- For Our use: Psychiatric Evaluation and H. Pylori Breath

Other Plans
Cigna, Aetna,
and other individual insurance companies will vary depending on
each individual policy. To find out the requirements, call you insurance
company with your patient ID number and ask what the policy coverage
criteria is based on the individual group plan.
BMI Calculator
*H. Pylori is a bacteria that has been implicated
in causing gastric ulcers. If found to be positive for the bacteria,
you will have to undergo 2 weeks of oral antibiotics which may
delay your surgery.
